I can probably clarify this. Drill Dozer was originally only released in Japan and North America, with the only supported language in the latter version being English. A European release was also planned at the time (based on the build date present in the EU ROM), which also supported French, German, Italian and Spanish. But for some reason it was never released until the game was put on Virtual Console.
So what this patch does is patch those additional four official localizations by Nintendo into the game. Or more accurately, it transforms the US ROM into a different ROM entirely, as the patch format used here is IPS which leads to the patch containing pretty much the entirety of the EU ROM's data. It would probably be better to replace it with something like an xdelta patch which can properly handle shifted data.
No clue on what the "RAM patches for rumble" would refer to though. Wii U RAM dumps show the ROM isn't being soft patched in the VC emulator. GBA VC ROMs are always hard patched.
